Community Habilitation and Supports (Community Hab) helps people with developmental disabilities build independence, confidence, and meaningful connections in their communities. Our individualized services support self-care, communication, socialization, daily living, and community living skills through activities such as educational programs, hobbies, community events, volunteer opportunities, and time with family and friends.
Services can be provided individually or in groups based on each person’s needs and authorized plan, with a focus on developing skills, self-advocacy, and greater participation in community life.
Day Habilitation – Individual (DHI)
